Student Demographics: Medical Info

The information on this Student Demographics page [>>] tab contains some information that is also found on the student's Health Profile page, as well as the following:

= Medical number: Depending on district policy, this may be a district-assigned medical number or a parent's medical plan number.
= Disability: Indicates that the student has a disability, such as hearing impaired.
= Critical/chronic health indicator: Indicates that the student has a chronic or potentially life-threatening condition.
= Medical alert: Indicates a significant medical condition, such as food allergy, asthma, or epilepsy. Note: If a medical alert is recorded here, Chancery SMS displays a Medical Alert icon at the top of the Student Demographics page [>>]. The alert does not specify the condition.
= Medical alert contact: The name and phone number of the school contact designated to provide health information for students.
= Permission to: Indicates permission to call a doctor, call an ambulance, or treat an illness or injury.
= Doctor's name: The name of the student's doctor.
= Doctor's phone: The doctor's phone number.
= Doctor's address: The doctor's business address.
